I agree to the following:

1. I have a valid voucher and agree to note BOTH numbers in the appointment notes.
2. If I fail to specify a valid voucher number, I am indicating I will be paying full price for this appointment, and I will not be able to use this voucher as payment. I may use it for a future visit, provided the terms on the voucher allow for that.
3. I agree to the cancellation policy which may require I pay for a service I did not receive if I fail to give proper notification of a missed service.
